Automated System Failure Allegedly Contributed to Fatal Train crash in Wales
On October 21, 2024, a tragic train crash in mid-Wales has raised serious questions regarding the safety systems in place. Allegedly, an automated system designed to help train wheels grip the tracks failed, leading to a collision between two Transport for Wales (TfW) trains. According to investigators, the hoses meant to discharge sand during braking were reported as “blocked,” which could have prevented the train from slowing down effectively.
The incident unfolded when one train, traveling at speeds between 15mph and 24mph, collided head-on with another moving at around 6mph. This collision resulted in the death of 66-year-old passenger David Tudor Evans and left four others seriously injured, with an additional 11 requiring hospital treatment. Both trains involved were two-carriage class 158 units, and the train that malfunctioned was supposed to stop in a loop to allow another train to pass on the single track.

Initial data from the onboard recorder revealed that the driver attempted to brake as the train approached the loop, but the wheels began to slide. Despite an emergency brake being applied about 40 seconds later, the train continued to slide for roughly 900 meters before the crash occurred.
The Rail accident Investigation Branch (RAIB) has stated that its investigation will thoroughly examine the failed sanding system, the performance of both trains, and the protocols of TfW and Network Rail regarding the management of wheel sliding risks. Following the incident, Network Rail noted that a railhead treatment train had run along the line the night before, which is crucial in preventing autumn leaf-related disruptions that can lead to slippery tracks.
In a joint statement, TfW and Network Rail emphasized their cooperation with ongoing investigations, insisting that the safety of passengers remains their top priority. However, the call for an independent inquiry into the incident has been echoed by friends of Mr. Evans, who are concerned about the apparent failings in safety systems. The community mourns a life lost, and many are left questioning how such preventable tragedies can occur.
