Kate Robertson, assistant coroner for North Wales East and Central, determined that the medical cause of death was methadone toxicity, highlighting the dangers of substance misuse.

David Fisher, who had previously served in the Armed Forces, had been battling substance misuse issues for several years. He had been known to the substance misuse service in Denbighshire and was prescribed methadone to help manage his addiction. Tragically, he was found unresponsive and was pronounced deceased shortly after.
