Tragic Accident Claims Lives of Two Workers in Northern Kentucky

BROMLEY, Ky. – The Kenton County Emergency Communications Center has released the dispatch report and 911 phone calls from a devastating job site accident that resulted in the death of two workers. The victims, identified as 23-year-old Mason Neises and 26-year-old Josh Mason, were contractors with Building Crafts, Inc. in Wilder, Kentucky.

The incident occurred at the Bromley Sanitation Facility when a plug on a pipe burst, causing a large tank to fill rapidly with water. Three individuals were inside the tank when the accident happened.

According to the 911 calls, one of the callers urgently requested rescue crews, explaining that the workers were submerged in the tank. The dispatcher struggled to understand the situation due to the caller’s distress.

The caller clarified that a plug in a “big concrete tank” had come loose, leading to the engulfment of Mason and Neises by the rapidly filling water.

The dispatcher asked if there were any air pockets inside the tank, to which the caller responded negatively, leaving no hope for the trapped workers’ survival.

Timeline of Events

The following is a timeline of the events leading up to the tragic outcome:

  • The first emergency call was received at 11:54 a.m. on December 14.
  • Four minutes later, it was reported that one person had been rescued, while two remained trapped in the tank.
  • The Occupational Safety and Health Administration (OSHA) was notified at 12:21 p.m.
  • At 12:23 p.m., a helmet was discovered washed through the filters, followed by a boot at 12:32 p.m.
  • The rescue team entered the tank at 5:08 p.m.
  • The first person was successfully removed at 6:13 p.m.
  • At 9:46 p.m., the first diver entered the water, followed by the second diver at 9:51 p.m.
  • The second person was located at 10:54 p.m.
  • At 11:11 p.m., divers recovered the second person.
  • Both divers were out of the water by 11:28 p.m.
  • The scene was finally cleared at 12:31 a.m. on December 15.

Multiple agencies, including the Ludlow Fire Department, Ludlow Police Department, Emergency Management crews, Kenton County Police Department, Covington Fire Department, Ft. Mitchell Fire Department, Edgewood Fire Department, Air Medical, Park Hills Police Department, the Kenton County Coroner’s Office, Villa Hills Police Department, and the Kenton County Sheriff’s Office, were involved in the rescue and recovery operations.
